Eligibility
- Must be an off-reserve Band member who received 2nd dose of vaccination
- Must be a Band Member of the age of 12 or more

Each application must include copy of the applicant’s COVID-19 Proof of Vaccination certificate issued by the
Saskatchewan Health Authority. An application will be deemed incomplete without the certificate.
